Perineal scars and vaginal repair after childbirth

For women after one or more vaginal births: understand scar pain, discomfort during sex, pelvic floor symptoms and the options before considering surgery.

This guide summarizes the main points of our Chinese article. Confirm your personal test preparation and treatment instructions with the clinical team.

Does every mother need surgery?

No. Childbirth history or appearance alone does not establish a need for surgery. Assessment should focus on your symptoms and wishes, not an assumption that tighter is better.

What is assessed?

Discuss tears or episiotomy, healing, breastfeeding, pain, dryness, urinary or bowel leakage, and a bulging sensation. With your consent, examination may assess scars, granulation tissue, muscle contraction and relaxation, and pelvic support. You can request a chaperone or pause the examination.

Non-surgical care

Treatment depends on the cause: wound problems, dryness, painful scars and muscle tension need different approaches. Pelvic health physiotherapy may help; strengthening alone is not suitable for every painful pelvic floor. Scar massage should wait until fully healed and clinically cleared.

Repair, risks and future pregnancy

Persistent symptomatic scarring or a structural defect may warrant discussion of targeted repair after assessment and appropriate conservative treatment. Ask exactly what is being repaired. Surgery can cause bleeding, infection, new scarring, altered sensation or painful sex and may require revision. It cannot guarantee sexual satisfaction or relief of all symptoms. Timing, recovery and future birth plans must be individualized.

Energy treatments and urgent symptoms

Laser or other energy treatments marketed as rejuvenation are not equivalent to repair surgery; benefit and long-term safety have limitations, and burns or persistent pain can occur. Seek prompt care for worsening pain, fever, pus, wound separation, difficulty urinating or new bowel leakage. Heavy bleeding or severe illness requires urgent care.
